How to Use This Speech-Language Pathology Calculator — Step-by-Step Guide
This free SLP productivity calculator is built for speech-language pathologists, CF-SLPs, and rehab directors. Three essential clinical and operational modules — 100% in-browser, no data stored.
Module 1 — Diagnostic & Clinical Metrics
Three sub-calculators for clinical assessment:
- Chronological Age: Exact age in years/months/days with proper "borrowing" logic. Optional prematurity adjustment (applied automatically when age < 24 months).
- MLU & PGU: Enter utterances, morphemes, words, and grammatical utterances. Calculates MLU-m, MLU-w, and Percent Grammatical Utterances.
- Fluency: Percent Syllables Stuttered (%SS) with severity classification, plus Syllables Per Minute (SPM).
Module 2 — PDPM SLP Case-Mix Classification
Check applicable conditions across two tiers. The calculator assigns the correct case-mix group (SA through SL) using the PDPM classification matrix:
- First Tier (0–3): Acute neurologic condition, SLP-related comorbidity, cognitive impairment
- Second Tier: Swallowing disorder, mechanically altered diet (Neither / Either / Both)
Module 3 — Billing & Compliance
- Mixed CPT Billing: Untimed codes (92507, 92521–92524) = 1 unit each. Timed codes follow the Medicare 8-minute rule.
- 25% Compliance Check: Verifies concurrent + group therapy ≤ 25% of total therapy minutes.
Chronological Age Calculator with Borrowing Logic — SLP Assessment
If days < 0 → borrow 30 days from months
If months < 0 → borrow 12 months from years
Example
- DOB: March 25, 2022 | Test: February 10, 2026
- Days: 10 − 25 = −15 → borrow → 30 + (−15) = 15 days, months reduced by 1
- Months: 2 − 3 − 1 = −2 → borrow → 12 + (−2) = 10 months, years reduced by 1
- Years: 2026 − 2022 − 1 = 3
- Result: 3 years, 10 months, 15 days
Mean Length of Utterance (MLU) & Percent Grammatical Utterances (PGU)
MLU-w = Total Words ÷ Total Utterances
PGU = (Grammatical Utterances ÷ Total Utterances) × 100
MLU-m is the gold standard for assessing expressive language development. Brown's Stages provide normative reference: Stage I (MLU 1.0–2.0, ~12–26 months), Stage II (2.0–2.5), Stage III (2.5–3.0), Stage IV (3.0–3.75), Stage V+ (3.75+).
PDPM SLP Case-Mix Classification Matrix (SA through SL)
| First Tier | Neither | Either | Both |
|---|---|---|---|
| 0 | SA | SB | SC |
| 1 | SD | SE | SF |
| 2 | SG | SH | SI |
| 3 | SJ | SK | SL |
Percent Syllables Stuttered (%SS) — Fluency Severity Classification
| %SS Range | Severity |
|---|---|
| < 3% | Mild |
| 3 – 8% | Mild-Moderate |
| 8 – 12% | Moderate |
| 12 – 25% | Moderate-Severe |
| > 25% | Severe |
Frequently Asked Questions
What is MLU and how is it calculated?
Mean Length of Utterance (MLU) divides total morphemes (MLU-m) or total words (MLU-w) by total utterances. MLU-m is the gold standard for assessing expressive language in children.
How does the PDPM SLP case-mix classification work?
PDPM classifies SLP patients into 12 groups (SA–SL). First Tier counts neurologic, comorbidity, and cognitive conditions (0–3). Second Tier evaluates swallowing disorder and mechanically altered diet. The combination determines the case-mix group.
What is the Medicare 8-minute rule for SLP billing?
Timed SLP codes (97129, 97130, 97530, 92526) follow the 8-minute rule: sum minutes, divide by 15, add 1 unit if remainder ≥ 8. Untimed codes (92507, 92521–92524) are always 1 unit each.
What is Percent Syllables Stuttered (%SS)?
%SS = (Stuttered Syllables ÷ Total Syllables) × 100. Severity: <3% mild, 3–8% mild-moderate, 8–12% moderate, 12–25% moderate-severe, >25% severe.
Is this calculator HIPAA-compliant?
Yes. All calculations run entirely in your browser with no patient data stored, transmitted, or logged.
